Institutional Registration & Application Form
- Scheduling. All Pre Conference Workshops (PCW) must be conducted on or before 21st August 2026, prior to the main conference dates (22nd & 23rd August 2026)
- Application Deadline. We kindly ask that applications reach us by 5th August 2026. This gives the Secretariat enough time to put together the documentation, accreditation, scheduling, and promotional flyers for your workshop.
Applications arriving after this date may be difficult to accommodate, so earlier is always better. - Fee Structure & PMS Share. The host institution / organiser is responsible for collecting workshop fees from participants.
50% of the total amount generated from participant fees must be remitted to the official Pakistan Menopause Society bank account (Section E).
Proof of this transfer must accompany after workshop. - Post-Workshop Reporting. Within one week following the workshop, the organiser must share with PMS:
(a) the final list of participants, and (b) documentary proof of the 50% amount remitted to PMS. - Certification. Certificates for participants, organisers, and facilitators are issued once the Secretariat has received and confirmed the participant list and proof of payment
- Depending on the workshop category and the relevant accreditation charges, CME points (where applicable) may be awarded by the Host Institution or facilitated through the PMS Secretariat
- Accuracy of Information. By signing, you’re confirming that the information provided is accurate and that your institution is happy to take part in the PMS Pre-Conference Workshops. in the PMS Pre-Conference Workshops.
- E-Signature for Certificates. Please email the Chief Organiser’s e-signature along with this form, so we can add it to the certificates.
